So what do you think? Does Zoidberg have a medical degree and forget everything that he has learned or did he never have one? And if he never had one how could he take the place of a doctor? Did he get the title from Prof. Farnsworth who'd hire anybody that's cheap?

Maybe Zoidberg is his people foremost expert on humans, and thus holds a doctor-degree in humans from the "Crustacean Institute of Xenobiology".

He could be a doctor among his own people, how hard could it be when the entire population dies every few decades or whatever, none of them would have much past experience, so any of them could be an expert compared to the others with a minimum of effort.

Zoidberg is a good umlisenced doctor. In 'Put your head on my shoulders' he saved Fry's head and also fixed up his mangled body.

I think Zoidberg has something against the freedom he has back on his homeplanet, which is why he feels at home on Earth.

He's a doctor for his own spieces obviously.  He's completely incompetant and bumbling for one thing.  As for a medical degree, he probably lost it.  Besides, Zoidberg even forgot that Fry had bones at one point. 

Also, Dr. Zoidberg is the punching bag of Futurama.

On his note paper it says "From the desk of Dr. John Zoidberg, M.D.". So he may actually have a right to bear the title of a doctor. Perhaps he bought it, which would explain why he's so desperately poor...

Wild thought here, but maybe he got his job the same way Fry, Leela, and Bender got theirs.  That is, the Professor needed a doctor and had a previously deceased doctor's career chip lying around.

This would explain why Hermes and Amy (and Bazuzu) seem to have gotten their jobs legitimately (through the University of Mars?) while others, such as Fry, Bender, and ZOIDberg, seem to have just shown up at the right time.
