I know Fry lives in bender's "closet" at robot arms apartments, the professor lives in some sort of bedroom in the tower of planet express, and hermes has his little house at wherever, but is it ever mentioned where the other characters live? Also, is the planet express building modeled in the game accurate? Is there any sort of sight with a complete map of the building? I want to do a half-life mod.

Zoidberg lives in alleys and dumpsters, and Leela lives in an apartment somewhere.

yeah, leela lives in apartment 1I, and i think zoidberg doesn't own a house, and is a hobo. Scruffy lives in the boiler room, or the basement some where, amy might live with her parents, or dorm or something, they only ever mentioned it in mars university, and i haven't seen it in a long time, but do you know where she lives when she's working and not in school, with her parents maybe, like in WTBR

Amy lives in an apartment building that seems impractical...if you walk out the front door, you'll fall to your death.

Zoidberg lives with Morbo in New Greenwich Village(unbeknownst to Morbo's wife)

Everyone lives in normal apartment, except for Professor and Zoidberg who lives in Profs genetical crossbreeding analyzer (KGKUAN).

Bender has an apartment at the Robot Arms and Fry lives in his closet.

Amy lives in an expensive looking high rise, as seen in "Put You Head On My Shoulders".

Leela has a spartan apartment (1i) in a crappy building.

Hermes lives with his wife, their home is briefly glimpsed in "Route of All Evil".

The Professor and Zoidberg live in the Planet Express building.

Cubert and Dwayne stay at the same boarding school for most of the year.

It's unknown where Scruffy lives, although he takes naps in the basement.

Have I left anyone out?

Zoidberg lives in the hydro-electrical region of the PE building (in that DNA spinning machine shown in 'Kiff Gets Knocked Up A Notch')
 it's like a window arch of the building, that arches into the Hudson River

Yeah, the café is diagonally across from the PE building.  Pottery Barn is probably directly across from the front door, like you said.

But Season 3 is befor the comics so he could have got it after that. maybe the coupon inspired him.

Why not? They're made by some of the show's crew, ex.:
