The supreme mutant had some surgery done?

In the first episode of the second season, "I Second That Emotion" the mutant with a third arm on his head is said to have only one ear. While in the second episode of the fourth season, "Leela's Homeworld" the very same mutant has two ears. His third arm has been elevated one side of his skull. Anyone else catch that?
