Scan a regular size framegrab, then resize the scan to 64*64

PEELers' FAQ: Avatars
PEELers' FAQ: UBB - to post images
PEEL File Uploader - Will host the file on the net and give you the UBB to put in your post.

They should all help point 1. For point two there are many guides, but basically:
  • Get frame grab
  • Blow up by 10x or whatever you feel is suitable
  • Scan image on a new layer
  • Delete the old layer
  • Crop and resize to 64x64.

If you want to make your own, check the first link as it has the requirements for avatars. If not, get the image you want made into an avatar and post it in the avatar thread and request someone make it, someone should be helpful.

Welcome to PEEL, enjoy your stay!

EDIT: Damn you Teral! That's what i get for being more informative is it, being beaten? Click my links, they're more useful and easier to understand!

Yes, that does. For me anyway. I am not sure about dimension_8, though.
