The normal Fry was confirmed to be called Phillip in The Problem With Popplers.

Then he was confirmed to be his grandfather in Roswell That Ends Well

Also, the famous Phillip Fry from Luck of The Fryrish.

Oh, and the Phillip Fry from Hovering Squidworld 97-A mentioned by the big brain for clafication in The Why Of Fry.

I think that's it...
